Home Health Workers
A Community Health Worker (CHW) or a Home Health Worker (HHW) is a public health worker who is a trusted member of and/or has an unusually close understanding of the community served. They may conduct assessments in health care settings, participants’ homes, and by phone. A health worker is critical in reducing health care costs, improving access to care, and advancing quality of care.
